Each standard deviation increase in CVAI corresponded to a 37% higher odds of CAS (OR 1.37, 95% CI 1.26-1.49). Crucially, Fasting Plasma Glucose (FPG) and Systolic Blood Pressure (SBP) were identified as parallel mediators, jointly explaining 59.2% of CVAI's total effect on CAS, with SBP contributing 36.0% and FPG 23.2%. A significant direct effect of 40.8% persisted. These findings highlight FPG and SBP as critical pathways linking visceral adiposity to atherosclerosis, suggesting that managing blood glucose and blood pressure is a core strategy for risk mitigation in individuals with elevated CVAI.
