Metacognitive Demands and Strategies While Using Off-The-Shelf AI Conversational Agents for Health Information Seeking

This research delves into the under-explored metacognitive demands individuals face when using off-the-shelf AI conversational agents (CAs) for health information seeking. Despite the convenience of consolidated responses, users grapple with significant mental effort to evaluate information, manage trust, and articulate their needs effectively. The study identifies specific challenges across prompt formulation, evaluation, iteration, and workflow adaptation.

Unlocking Deeper Engagement: Navigating Metacognitive Demands in AI Health Information Seeking

Key findings highlight that users struggle with initial prompt phrasing, often resorting to 'trial questions' to test agent trustworthiness or using broad, impersonal queries to maintain privacy. Evaluating verbose and often overly confident AI responses demands careful monitoring for relevance and accuracy, with many users employing strategies like focusing on bolded text or summaries. Iteration is a common but often frustrating process, as users attempt to clarify symptoms or rephrase questions, sometimes getting stuck in unproductive loops or offloading the task to the agent.

Crucially, the study reveals a constant need for users to maintain self-awareness about the AI's role—distinguishing between information gathering and medical advice—and to adapt their workflow when the agent's conversational flow diverges from their goals. Based on these insights, five design considerations are proposed for future CA interfaces, focusing on scaffolding goal-setting, structuring prompts, safeguarding sensitive disclosures, ensuring input transparency, and enhancing information evaluation through visual and reflective tools. These considerations aim to reduce metacognitive demands, promote safer interactions, and improve the overall user experience in health information seeking.

Users face significant challenges in articulating their health queries effectively, including self-awareness of health goals and risks of information disclosure and task decomposition of symptoms and health queries. They often struggle to decide what details to provide and how to phrase them to ensure the AI understands correctly, leading to a demand for well-adjusted confidence in symptom description.

After receiving AI responses, users grapple with evaluating the information. This involves a constant need for well-adjusted confidence in weighing trustworthiness of health responses, as they assess the accuracy, completeness, and relevance of the AI's output, often feeling uncertain due to the overly confident tone or lack of source citations.

Refining queries based on previous responses demands well-adjusted confidence in clarifying symptoms. Users experiment with different phrasings to ensure understanding, but can get caught in unproductive loops. This highlights the need for metacognitive flexibility to break rephrasing illness episodes and know when to change strategy or offload tasks to the agent.

Users must recognize and adapt their interaction workflow, which requires self-awareness of the agent's role in health decisions—understanding its limits and not letting it drive critical health decisions. It also involves well-adjusted confidence in relying on the agent without letting it drive health decisions and metacognitive flexibility in adjusting workflow strategies in response to agent-led shifts when the AI steers the conversation off-course.

User Strategies for Evaluating AI Responses

Strategy Benefits Limitations
Focus on Bolded Headings/Summaries
  • Quickly identifies key points.
  • Reduces information overload.
  • May miss crucial details.
  • Risk of oversimplification.
Cross-checking with Trusted Sources
  • Enhances trustworthiness.
  • Validates AI's accuracy.
  • Time-consuming and interrupts flow.
  • Requires external digital literacy.
Rewriting Agent's Replies into Personal Notes
  • Aids comprehension and recall.
  • Helps identify missing information.
  • Manual effort, especially for long responses.
  • Can be tedious and inefficient.

Navigating AI's Overly Confident Tone

Context: One participant, P11, struggled with the AI's overly confident tone, stating, 'It sounds so sure, but I don't know if it's right.' This sentiment was common among first-time AI users, leading to uncertainty about the source and generation of information.

Challenge: The AI provided no clear signals on what to trust or how to weigh information, forcing users into extensive self-evaluation. This constant demand for 'well-adjusted confidence' put a significant cognitive load on users, who had to balance trust with vigilance against misleading information.

Solution: Design considerations recommend transparent output, confidence cues, and uncertainty visualization to help users monitor and review AI output more effectively. This could include showing sources and indicating confidence levels for different pieces of information, allowing users to better manage their trust and make informed decisions.

Result: By implementing such features, users could potentially experience a more transparent and less cognitively demanding evaluation process, fostering a healthier human-AI interaction in sensitive health contexts.

Your Strategic AI Implementation Roadmap

A phased approach to integrate metacognition-aware AI conversational agents into your enterprise for health information seeking.

Phase 1: Discovery & Needs Assessment

Conduct workshops with key stakeholders to understand existing health information seeking behaviors, pain points, and desired outcomes. Define specific use cases where AI CAs can provide the most value.

Phase 2: Pilot Program & Customization

Deploy a pilot AI CA solution with enhanced metacognitive scaffolding (e.g., goal-setting, prompt structuring tools) for a select group of users. Gather feedback and customize the agent's responses and interface to align with organizational needs and health literacy levels.

Phase 3: Training & Rollout

Develop comprehensive training materials for employees on how to effectively use the AI CA, including strategies for prompt formulation, evaluation, and understanding AI limitations. Gradually roll out the solution across the organization.

Phase 4: Monitoring & Continuous Improvement

Establish a monitoring framework to track AI CA usage, user satisfaction, and the impact on health information seeking efficiency and accuracy. Continuously refine the AI agent and its features based on user feedback and emerging health information needs.
