Our study confirms that different pain levels strongly correlate with spectral features, including pitch, formants, and energy distribution in high-frequency bands. Physiological changes due to pain—such as tightening throat muscles or increased air pressure from lungs—manifest as quantifiable shifts in vocal signals. For instance, higher pain levels show more energy in high-frequency ranges and a broader spectrum, resulting in a higher perceived pitch. We introduce a low-cost, real-time framework for classifying pain levels (low, moderate, high) using acoustic sensors and pre-trained Convolutional Neural Network (CNN) models. This system automates feature extraction from audio streams, identifying spectral features like Mel-Frequency Cepstral Coefficients (MFCCs), pitch, and formant frequencies. Designed for non-clinical, resource-constrained settings, it minimizes computational complexity, making it suitable for edge deployments like Raspberry Pi devices. Our CNN model achieves an average accuracy of 72.74% for three-level pain classification, outperforming existing binary methods by 18.94–26.74%. When trained with verbal audio (TAME dataset), it significantly surpasses nonverbal audio (VIVAE dataset) performance (55.56%). The system demonstrates comparable false negative rates for low and high pain with facial-expression methods. We also observed that classification accuracy improves with more training samples and that biased sample distributions can lead to reduced sensitivity for underrepresented pain levels.
The lightweight design and low computational requirements make our framework ideal for real-world deployment in sensor-assisted intelligent care, particularly home-based and independent living scenarios. It processes voice signals locally to preserve privacy and allows for system updates and model refinement.
